Cutaneous Aspergillosis in the context of AIDS

Summary:

A 52 year old Caucasian male with a history of AIDS (recent CD4 count 35/mm3, viral RNA 66,000 copies/ml) and CMV meningoencephalitis, developed culture-proven, cutaneous aspergillosis. He was diagnosed based on repeated culture of several lesions. He was treated with caspofungin for 39 days, as he was intolerant to Amphotericin B (severe systemic infusion reaction).
The patient has known AIDS for which he was treated with indinavir, ritonavir, and abacavir. He also developed CMV meningoencephalitis for which he was maintained on ganciclovir since January 2000. Other medications included trimethoprim-sulfamethoxazole, azithromycin, and ethambutol for PCP and MAI prophylaxis.
