Acute Disseminated Aspergillosis following lung transplantation for cystic fibrosis

Summary:

This 52 year old male with a history of cystic fibrosis, and bilateral lung transplantation in July 1997, developed probable pulmonary aspergillosis (based on BAL culture and chest CT). He was both intolerant of Amphotericin B deoxycholate (creatinine 2mg/Dl) and refractory to Abelcet (with progression of disease). He was treated thereafter with 2 days of caspofungin, but he quickly expired from respiratory failure with progression of lung disease and probable cerebral dissemination.
