Subacute invasive pulmonary aspergillosis and AML

Summary: 

This 69 year old white female with recently diagnosed acute myeloid leukaemia (AML) developed definite pulmonary aspergillosis during her second cycle of chemotherapy. She was diagnosed based on chest CT (sub-pleural consolidations) and an open lung biopsy showed hyphae in lung tissue. She was refractory to Abelcet (with progression of disease) and itraconazole (abnormal liver function tests). She was treated with caspofungin for 37 days with good response, the patient was not neutropenic during this time.
